Billing for Medication Management
Complex medication management is a cornerstone of internal medicine, and this work is a key component of your E/M code selection. The number and complexity of prescriptions you manage, along with the risk associated with those medications, directly contribute to your Medical Decision Making (MDM). Be sure to document your review of medications, refills provided, and any changes to the patient's regimen to support the level of E/M service billed.
Laboratory Testing and Interpretation
If your office is CLIA-certified to perform in-house labs, you can bill for those tests directly. More commonly, practices bill for the interpretation of outside lab results. This cognitive work is not separately billable but is another critical factor in determining your MDM level for an E/M visit. Documenting "reviewed labs" is not enough; your note should specify which labs were reviewed and how they influenced your treatment plan.
Data Points Matter: Under the MDM guidelines, each unique lab test reviewed counts as a "data point." The total number of data points is a major factor in justifying moderate or high-complexity E/M codes.
